Worked Example
- Compute pre-pregnancy BMI. 62 ÷ 1.65² = 22.8 — the normal-weight band.
- Read the total target. Normal weight: 11.5 to 16 kg over the pregnancy.
- Add the first trimester. 0.5 to 2 kg, the same for every BMI category.
- Add the weekly rate. 0.35–0.50 kg/week × (24 − 13) = 3.9 to 5.5 kg.
- Compare. Expected 4.4 to 7.5 kg; actual 6.5 kg — on track.
Two features of the IOM guidance are routinely got wrong. The first is that everything follows from the PRE-pregnancy BMI, not from current weight — which is why an accurate starting figure matters more than any later measurement, and why clinicians substitute the first-trimester weight when it is unavailable. The second is that the weekly rates apply only from around week 14. The first trimester is treated as a flat 0.5 to 2 kg for every category, because early gain is small and highly variable; nausea means some women lose weight in that period, which is common and usually not a concern. Applying a weekly rate from week one, as some trackers do, overstates the expected figure and can generate unnecessary alarm. A single measurement also means little on its own — the trend across appointments is what your midwife is reading.

Gestational weight gain is assessed against the 2009 Institute of Medicine guidelines, which define total gain ranges and second and third trimester weekly rates according to pre-pregnancy body mass index in the four World Health Organization categories. The engine computes that index from pre-pregnancy weight and height, since the entire recommendation follows from it, and applies the corresponding total range of 12.5 to 18 kilograms for underweight, 11.5 to 16 for normal weight, 7 to 11.5 for overweight and 5 to 9 for obese women. Expected gain at a given gestational week is constructed in two parts, reflecting the structure of the guidelines themselves: a flat allowance of 0.5 to 2 kilograms across the entire first trimester, applied identically to every category because early gain is small and highly variable, followed by the category-specific weekly rate from approximately week fourteen. Applying a weekly rate from conception, as some trackers do, systematically overstates expected gain. Provisional twin ranges are carried where the IOM published them, with the engine stating explicitly that no guidance exists for underweight women carrying twins and that twin figures reflect observed rather than trial-tested ranges.
