Worked Example
- Measure the waist correctly. 85 cm, taken midway between the lowest rib and the top of the hip bone.
- Divide by height. 85 ÷ 170 = 0.500.
- Read the band. 0.500 sits exactly at the start of the increased central adiposity band.
- Find the healthy ceiling. 170 × 0.5 = 85 cm, so the waist must come below that.
- Compare against BMI. 72 kg at 1.70 m is a BMI of 24.9 — normal, and misleading here.
This worked case is the entire argument for the measure. A BMI of 24.9 sits comfortably inside the normal range and would end the conversation in most check-ups, while the waist ratio lands exactly on 0.500 and says something different. Analysis of UK survey data found roughly a third of adults classed at no increased risk by a BMI-and-waist matrix actually had a ratio of 0.5 or above, and that group carried measurably worse cardiometabolic risk factors even after adjusting for BMI, age and sex. The reason is anatomical rather than statistical: BMI knows your total mass but nothing about where it sits, and visceral fat around the liver and pancreas behaves very differently from the same mass carried on the hips. One caution on precision — at exactly 0.500 you are one or two centimetres of tape technique from a different band, so measure consistently before drawing conclusions.

Waist-to-height ratio divides waist circumference by height in the same units, so the result is dimensionless and the underlying instruction reduces to keeping the waist below half the height. This engine bands the result according to NICE guideline NG246, which classifies 0.4 to 0.49 as healthy central adiposity, 0.5 to 0.59 as increased, and 0.6 or above as high, and which applies those thresholds to both sexes and all ethnicities in adults with a body mass index below 35, explicitly including adults with high muscle mass. The boundary of 0.5 was first proposed by Ashwell and Cole in 1996 as a proxy for central adiposity correlated with abdominal imaging.
