What this result does not account for

Worked Example
- Estimate VLDL. 180 ÷ 5 = 36.0 mg/dL, carried by triglycerides.
- Back out LDL. 213 − 48 − 36.0 = 129.0 mg/dL.
- Check the decomposition. 129.0 + 48 + 36.0 = 213.0, reconciling exactly to the stated total.
- Find non-HDL. 213 − 48 = 165 mg/dL — every atherogenic particle in one figure.
- Compare an identical total. The same 213 with HDL 75 and TG 75 gives non-HDL of 138 — 27 mg/dL less atherogenic cholesterol.
The comparison at the bottom of this page is the whole argument. Three people can each have a total cholesterol of 213 mg/dL and be in entirely different positions: with HDL 75 and triglycerides 75 the TC/HDL ratio is 2.84 and non-HDL is 138; with HDL 32 and triglycerides 280 the ratio is 6.66 and non-HDL is 181. Same headline number, more than double the ratio. Total cholesterol is a sum of components that move in opposite directions, so it can improve for bad reasons and worsen for good ones — which is exactly why no modern guideline sets a total cholesterol target and why a screening result that quotes only the total cannot be acted on.

Total cholesterol is the sum of the cholesterol carried by every lipoprotein fraction, and this engine decomposes it into its parts rather than interpreting the total in isolation. VLDL is estimated as triglycerides divided by five, the same assumption underlying the Friedewald equation and subject to the same limitations as triglycerides rise; LDL is then obtained as the remainder. The decomposition is shown reconciling to the stated total so the arithmetic can be checked directly. The substantive point the engine makes is that the total is a sum of components that move in opposite directions with respect to risk: an increase driven by HDL is favourable while an identical increase driven by LDL is not, so the same movement in the headline figure can mean opposite things. To make that concrete the engine recomputes the entire panel for a second person sharing the identical total but different HDL and triglycerides, and reports the resulting difference in non-HDL cholesterol and in the total-to-HDL ratio. Non-HDL is highlighted throughout because it requires no equation, makes no assumption about triglycerides, and captures every atherogenic particle, which is why contemporary guidance has moved toward it and away from total cholesterol, for which no modern target exists.
