Ratios compress two numbers into one and lose the magnitude doing it. That is convenient for description and misleading for targeting.

Worked Example

  1. Divide total by HDL. 213 ÷ 48 = 4.44 — acceptable, but above the 3.5 optimum.
  2. Compute the tighter ratio. 129.0 ÷ 48 = 2.69, in the moderate 2.0 to 3.0 band.
  3. Check the metabolic marker. 180 ÷ 48 = 3.75, above the 3.0 mark that flags insulin resistance.
  4. Convert the LDL goal to non-HDL. A 100 mg/dL LDL goal implies a non-HDL target of 130 mg/dL.
  5. Measure the real gap. Non-HDL of 165 against a 130 target is 35 mg/dL — the number worth acting on.

The trap on this page deserves stating plainly. A ratio improves if the numerator falls or the denominator rises, and those are not equivalent events. Raising HDL by 10 mg/dL moves the TC/HDL ratio from 4.44 to 3.67, an apparent 17 percent improvement, while leaving every single atherogenic particle exactly where it was — non-HDL does not move at all. The drug trials that achieved precisely that kind of HDL increase produced no reduction in events. A ratio cannot distinguish a real improvement from a cosmetic one, which is why guidelines now set targets against non-HDL: it keeps the absolute magnitude, needs no division, and responds to the treatments that actually work.

The total-to-HDL ratio is the classic summary measure, with conventional bands placing below 3.5 as optimal and above 5.0 as elevated. The LDL-to-HDL ratio is tighter because it excludes the VLDL fraction contributed by triglycerides. The triglyceride-to-HDL ratio is not a cholesterol ratio at all but a metabolic marker, widely used at a threshold of 3.0 as an indicator of insulin resistance and of a small dense LDL pattern that a standard panel cannot observe directly. Band verdicts are computed on the rounded displayed value so the card and the verdict cannot contradict each other. The engine gives substantial weight to the structural weakness of ratios as treatment targets: a ratio improves identically whether the atherogenic numerator falls or the HDL denominator rises, yet only the first reduces cardiovascular events, and the trials that raised HDL pharmacologically delivered no benefit.
