HDL needs no equation. What it needs is interpretation, because the relationship between HDL and risk is not linear and not causal.

Worked Example

  1. Place it against the sex threshold. 48 mg/dL is above the 40 mg/dL male risk mark but below the 60 mg/dL protective mark.
  2. Compute the share. 48 ÷ 213 = 22.5% of total cholesterol.
  3. Find what is left. 213 − 48 = 165 mg/dL of non-HDL — every atherogenic particle.
  4. Check the curve. 48 sits on the rising-benefit portion, well below the ~90 mg/dL point where mortality turns back up.
  5. Set a realistic expectation. Exercise moves HDL by roughly 5 to 10 percent, so a 12 mg/dL rise needs weight loss too.

HDL is the most misunderstood number on a lipid panel because the story attached to it — good cholesterol, more is better — is not what the evidence shows. Observational mortality is U-shaped, with the benefit flattening and then reversing above roughly 90 mg/dL. More decisively, every attempt to convert the association into a treatment has failed: CETP inhibitors raised HDL dramatically without reducing events, and niacin did the same. That is the signature of a marker rather than a mechanism. The practical consequence is that a low HDL should prompt attention to overall risk, but the number itself is not a target to chase — the 165 mg/dL of non-HDL sitting alongside it is where the treatable risk actually lives.

Thresholds are sex-specific: below 40 mg/dL in men and 50 mg/dL in women, a low HDL is conventionally counted as an independent cardiovascular risk factor, while 60 mg/dL or above has traditionally been treated as a negative risk factor offsetting one other. The engine reports HDL as a share of total cholesterol and, more usefully, reports the non-HDL remainder, since that fraction contains essentially every atherogenic particle and is what actually drives risk. Considerable weight is given to the non-linearity of the relationship, because the popular framing of HDL as good cholesterol misleads in two directions: observational mortality curves are U-shaped with excess mortality reappearing above roughly 90 mg/dL in men, and pharmacological HDL raising has repeatedly failed to reduce cardiovascular events despite achieving large increases, most notably with the CETP inhibitors and niacin.
