The margin here is two pack-years, which is uncomfortably thin, and that is the practical warning. Smoking histories are rarely uniform — people smoke more during stressful years and less during others — so a remembered average of twenty a day can easily be eighteen or twenty-four, and either moves this person across the eligibility line. The other point worth making is that the 2021 criteria are substantially wider than the ones they replaced: the threshold fell from 30 pack-years to 20 and the starting age from 55 to 50, explicitly because the older rule excluded too many women and Black smokers, who tend to develop lung cancer at lower cumulative exposure.
