Worked Example
- Estimate the total. 80 kg × 0.5 units/kg = 40 units a day.
- Take the basal share. 50% of 40 = 20 units of long-acting insulin.
- Leave the rest for meals. 20 units of bolus, divided by the carbohydrate in each meal.
- Run the overnight test. 140 mg/dL at bedtime, 175 on waking — a rise of 35 mg/dL.
- Read it carefully. 35 exceeds the 30 mg/dL band, but rule out a late meal and overnight rebound before increasing.
The overnight drift of 35 mg/dL sits just outside the acceptable band, and that is a deliberately awkward result. The naive reading is that basal is too low, so increase it. But a rise before waking is equally consistent with the dawn phenomenon, which is normal physiology, and with a rebound from an overnight low that was slept through — and those two call for opposite changes. At breakfast they look identical. Only a 3am reading or continuous monitoring separates them. This is why the rule is to test on two or three clean nights, not one, and why increasing basal on a single morning number is how people titrate themselves into nocturnal hypoglycaemia.
